one thing i didnt understand was, when he got to the island he met the "chubby brown haired" warden, then midway the warden changed to the blonde haired dude from "HEAT", what happened to the original?

first guy wasnt the warden, it was the deputy. Stick with it munky, your age is starting to show.

Hmm.. I figured out the main twist about 30 minutes in.. when you started noticing the little things like the chick he was interviewing asked for a glass of water and it shows her taking a drink but there's no glass. There's also a couple of scenes played in reverse (cigarette smoke was going the wrong direction, Teddy walking backwards) and a few odd camera tricks where the camera angle changes but the scene changes very noticeably between shots. These things all hinted that Teddy was having a break from reality. When the psychiatrist put forth the idea that the missing patient had created her own little fantasy world, I realized that it was actually Teddy's scenario. I love these kinds of movies, unfortunately this one was a bit predictable. Great acting though.

I'll recommend a few other mind-fuck movies along the same vein:

1) Memento
2) Mulholland Drive
3) Stay

That's why Memento is a brilliant film and gets better with every viewing. The first time you see it you realize a lot of cool shit happened but you didn't catch most of it. Then every time you watch it afterwords you pick up on more and more details. But you can never quite figure it out because Nolan purposefully left the ending ambiguous (which is actually the middle of the movie, chronologically).

If you thought Memento was hard to follow, then steer clear of a movie called Primer. It's about time travel and has 9 separate timelines to keep track of.. people have mapped it and it's fucked up:
